About the Job

The Veterans Affairs Northern California Health Care System is seeking a highly skilled Social Service Representative to join our team. As a key member of our social work service, you will play a vital role in providing support to homeless veterans and their families.

Job Summary

The Social Service Representative will work independently and in consultation with HUD-VASH Case Managers to provide information, arrange services, and support the HUD-VASH program. This includes conducting interviews, gathering information, and coordinating with internal and external customers to ensure veterans' healthcare needs are met.

Responsibilities
  • Provide or obtain information, including interviewing patients and families, to support the HUD-VASH program.
  • Consult with case managers on veteran-related questions and concerns.
  • Contact internal and external customers to ensure veterans' healthcare needs are met.
  • Interpret clinical material, such as standard operating instructions and advance directives.
  • Coordinate Homeless Program functions, including emergency procedures associated with the Clinical Command Center.
  • Gather administrative information and communicate it via email, written correspondence, or presentation at various committees.
  • Track and document all activities and requirements for the program assigned.
  • Assist in the development of standard operating procedures for programs used within the Clinical Command Center.
Requirements
  • U.S. citizenship is required.
  • Selective Service Registration is required for males born after 12/31/1959.
  • You may be required to serve a probationary period.
  • A valid driver's license is required.
  • Subject to background/security investigation.
  • Selected applicants will be required to complete an online onboarding process.
  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
  • Participation in the Coronavirus Disease 2019 (COVID-19) vaccination program is a requirement for all Veterans Health Administration Health Care Personnel (HCP).
Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 10/14/2024. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-9 position, you must have served 52 weeks at the GS-7.

There is no educational substitution at this grade level.

Additional Information

Receiving Service Credit or Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee. Selected applicants may qualify for credit toward annual leave accrual, based on prior work experience or military service experience.
