Job SummaryWe are seeking a skilled Physical Therapist Assistant to join our team of healthcare professionals. As a Physical Therapist Assistant, you will work under the supervision of a licensed Physical Therapist to provide physical therapy services to patients in their own homes.
Key Responsibilities- Assist in the development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
- Treat clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, light and electricity.
- Teach, supervise, and counsel the family and client in the physical therapy program.
- Instruct client in care and use of adaptive devices such as wheelchairs, braces, crutches, canes and prosthetic and orthotic devices.
- Arrange for out-patient service for procedures when appropriate.
- Record on a timely basis all treatments, and client's response to therapy intervention Also records quality assurance data.
- Communicate with PT and other team members (including Home Health Aides), when appropriate. Instruct them in certain phases of physical therapy that they may use while working with the client
- Under the direction of a Physical Therapist, write clinical/progress notes on the date of the visit and incorporate same in the client's record according to company policy.
- Participate in case conferences, physical therapy staff meetings, agency activities and in-service programs as necessary.
- Participate in effective coordination of schedule to ensure all reassessments are performed in a timely manner.
- Obtain a medical history, particularly as it relates to the present condition.
- Under the supervision of the physical therapist, assist in evaluation and development of a rehabilitative plan of care and periodic re-evaluation as required.
- As directed, perform routine strengthening and therapeutic exercises and assist the patient with the use of self-help devices.
- Identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication and report same to the PT.
- Observe, and record activities and findings in the clinical record and report to the PT and the physician any adverse reaction to treatment.
- Participate with staff, clients, and physicians in discharge activities and assist the Physical Therapist in completing the Physical Therapy Discharge Summary.
- Assume other duties deemed appropriate and necessary by the PT and as designated by Supervisor
- The ability to drive and the ability to access clients' homes, which may not be routinely wheelchair accessible are required.
- Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
- May be required to bend, stoop, reach and move client weight up to 250 pounds, lift and/or carry up to 30 pounds.
- Must be a graduate of an approved school giving a course of not less than two years for Physical Therapist Assistants.
- Must be licensed by the State as a Physical Therapist Assistant.
- Must have one year of experience in an acute/sub-acute setting.
