About the RoleWe are looking for a highly motivated and experienced Physical Therapist to administer physical therapy to patients on an intermittent basis in their place of residence. As a Registered Physical Therapist, you will work under the direction and supervision of the Director of Clinical Services/Clinical Manager to ensure that the highest quality care is always maintained.
Responsibilities- Administer physical therapy to patients in their place of residence
- Develop and implement individualized plans of care to meet the unique needs of each patient
- Conduct initial assessments, reassessments, and progress notes to ensure optimal patient outcomes
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Participate in quality assurance and performance improvement activities to continuously improve patient care
- Graduate from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education
- Currently licensed as a Registered Physical Therapist in the state(s) in which practicing
- Minimum of one year current experience in healthcare community, home health experience preferred
- Ability to exercise initiative and independent judgment
- Have a sympathetic attitude toward the care of the sick and elderly
