Key Responsibilities- Conduct comprehensive annual wellness exams and provide counseling and education to patients and families about benefits and programs available to improve their quality of life.
- Document patient information, including chief complaint, diagnosis, medical history, and plan of care.
- Coordinate care with primary care providers, specialists, and ancillary services to ensure seamless patient care.
- Deliver evidence-based, timely care that reduces avoidable hospitalizations and maximizes quality of life.
- Prescribe medications, order tests, and collaborate with patients' Monogram physicians to ensure comprehensive care.
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
- Board certification for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA certificate.
- Ability to work autonomously and practice without direct supervision.
- Access to transportation, a valid driver's license, and car insurance.
- Proficiency with medical instruments and equipment required by the work.
- Knowledge of computer-based data management programs and information systems.
