Key Responsibilities
  • Conduct comprehensive annual wellness exams and provide counseling and education to patients and families about benefits and programs available to improve their quality of life.
  • Document patient information, including chief complaint, diagnosis, medical history, and plan of care.
  • Coordinate care with primary care providers, specialists, and ancillary services to ensure seamless patient care.
  • Deliver evidence-based, timely care that reduces avoidable hospitalizations and maximizes quality of life.
  • Prescribe medications, order tests, and collaborate with patients' Monogram physicians to ensure comprehensive care.
Requirements
  • Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
  • Board certification for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
  • Current and unrestricted DEA certificate.
  • Ability to work autonomously and practice without direct supervision.
  • Access to transportation, a valid driver's license, and car insurance.
  • Proficiency with medical instruments and equipment required by the work.
  • Knowledge of computer-based data management programs and information systems.
