We are seeking a highly skilled Pharmacist to join our team at Mount Sinai Hospital. As a Pharmacist, you will play a critical role in ensuring the safe and effective use of medications for our patients.
Responsibilities- Dispense medications accurately and in a timely manner.
- Resolve questions regarding formulary and non-formulary medications, following established policies and procedures.
- Provide dosing and therapeutic recommendations based on patient age and kinetic considerations.
- Perform clinical chart reviews on a scheduled basis.
- Conduct Patient Care Unit inspections on a scheduled and timely basis.
- Prepare parenteral medications using sterile aseptic technique.
- Maintain adequate medication supply in the Pharmacy area and inform supervisor of impending shortages.
- Intervene and document interventions to correct or improve pharmacotherapy.
- Report adverse drug events and medication errors occurring in the area serviced.
- Take responsibility for the operation of the assigned Pharmacy area and activities of all supportive personnel.
- Maintain pharmacological knowledge by reading professional publications, attending workshops, conferences, and C.E. programs to maintain professional competency.
- Adhere to hospital dress code and maintain an acceptable attendance record.
