As a non-profit organization, UCP participates in the Federal Student Loan Forgiveness Program, potentially allowing full-time employees to have their remaining Direct Loan balances forgiven.

Key Responsibilities:
As a Lead Program Coordinator, you will:


Assess referrals to determine suitability for placement and collaborate with the Residential Manager and Director to evaluate the capacity to support individuals in residential services.

Act as the primary Specialist on the admissions review committee, evaluating each new referral for residential services.


Coordinate visits with potential referrals and provide feedback to the Director of Residential and Clinical Services regarding possible admissions.


Review and enhance documentation practices within residential services, assisting team members in maintaining timely and accurate records.


Develop and manage comprehensive assessments for each individual on your caseload, ensuring current and accurate evaluations of their skills, potential, and needs.


Create annual Personal Support Plans tailored to the specific needs and aspirations of each individual, collaborating with all team members.

Monitor the implementation and effectiveness of Personal Support Plans/Individual Support Plans.


Provide necessary assistance, training, and support to help individuals develop personal competencies and integrate into their communities.


Lead efforts to support team members in developing teaching strategies that promote independence, responsibility, and self-esteem for residents. These strategies will empower residents with choice and control.

Conduct regular audits of resident files on your caseload.

Communicate audit results to the Residential Manager, Residential Supervisor, and Residential Services Director.


Develop and implement new policies and procedures to enhance operational effectiveness and efficiency.

Complete all assessments and quarterly reports within designated timeframes.


Collaborate with the Director of Residential and Clinical Services to train new Specialists, ensuring compliance with program requirements and regulations regarding assessments, ISPs, Support Plans, and necessary protocols.


Prepare agendas and lead monthly Specialist meetings to address issues or concerns across the division.

Design and implement new training programs as needed to support the development of curriculum training for all Specialists, focusing on licensing or quality standards.

Qualifications:

Minimum Requirements:
Bachelor's degree with 5-7 years of relevant experience in a Program Specialist role.

Certification as an Enabling Technology Integration Specialist (SHIFT).

Charting the Lifecourse Ambassador badge.

A valid driver's license and access to a vehicle with current insurance, registration, and inspection.

Strong communication skills in English, including reading, writing, and interpretation.

Preferred Qualifications:
Familiarity with applicable compliance regulations (i.e., Chapter 51, 6400).

Experience in Developmental Planning and Goal Setting.

Multi-Site Management experience.

Knowledge of disability services funded through Medicaid and Pennsylvania's Department of Health and Human Services.
