VNS Health is seeking a skilled Registered Nurse to join our Home Health team in North Bergen, New Jersey. As a key member of our interdisciplinary care team, you will be responsible for providing high-quality patient care and support to seniors in their own homes.
Key Responsibilities- Collaborate with NORC site staff to ensure residents engage in self-care management health strategies.
- Provide health-focused outreach to seniors who may benefit from supportive services provided by the program.
- Implement health promotion activities, including health screening, health assessments, health care linkages, and health presentations.
- Make telephone calls and home visits to individual residents to assess their needs and support successful community living activities.
- Make referrals to appropriate health services based on the assessed needs of the resident.
- Provide healthcare advocacy by acting as a liaison between various healthcare systems during times of transition.
- Participate in the Quality Assurance Program and Education Program as requested by the Clinical Manager.
- License and current registration to practice as a Registered Professional Nurse in New York State.
- CPR/BLS Certification.
- Associate's Degree in nursing required; Bachelor's Degree in nursing preferred.
- Minimum of two years of experience working as an RN in LHCSA or CHHA.
- Prior experience working with an aging population and providing clinical assessments of their health needs.
