Job Summary

We are seeking a skilled Hospice Care Case Manager to join our team. As an experienced Registered Nurse, you will play a vital role in providing high-quality care to our patients and their families, ensuring their physical, emotional, and spiritual needs are met.

Key Responsibilities

  • Care Planning and Coordination
  1. Develop and implement comprehensive care plans in collaboration with the interdisciplinary team.
  2. Conduct ongoing assessments of patient needs and revise care plans as necessary.
Patient and Family Support
  1. Provide emotional support and education to patients and their families, addressing their physical, emotional, and spiritual needs.
  2. Counsel patients and their families on end-of-life care options and ensure their wishes are respected.
Collaboration and Communication
  1. Work effectively with the interdisciplinary team, including physicians, social workers, and other healthcare professionals.
  2. Communicate patient information and care plans to the team, ensuring seamless care transitions.

Requirements

  • Education and Experience
  1. Graduation from an accredited school of nursing.
  2. 1-2 years of recent acute care experience in an institutional setting (required). 1-2 years of recent home care experience (preferred).
Clinical Skills
  1. Ability to assess patient needs and develop effective care plans.
  2. Skilled in pain and symptom management, as well as end-of-life care.
Personal Qualities
  1. Empathy and compassion for patients and their families.
  2. Strong communication and interpersonal skills.
