Scope of Work

In collaboration with the interdisciplinary team, you will assess, plan, implement, coordinate, monitor, and evaluate the strategies and services required to meet a patient's end-of-life care needs. You will create an outcome-based plan of care, continually adjusting it through the utilization of the nursing process, relationship-centered care principles, and a holistic philosophy.

Responsibilities
  • Communicate effectively with staff, residents, and families
  • Assess, develop, and maintain an individualized patient plan of care
  • Perform medication administration, as needed
  • Communicate with other members of the healthcare team to ensure continuity of care and coordination of services
  • Evaluate patient's and significant other's educational needs and develop and implement appropriate teaching strategies
  • Identify problems and emergency situations, and initiate immediate life-saving measures
Qualifications
  • High School Diploma or equivalent
  • Completion of an approved nursing program
  • Associate's Degree in Nursing, preferred
  • 1 year of relevant experience in acute care, emergency, or rehab facility experience, preferred
  • AHA American Heart Basic Life Support certification
  • Licensed Practical Nurse (LPN) in the State of Michigan
