Position Overview: We are currently seeking a qualified Certified Hospice Caregiver to join our compassionate team in providing exceptional end-of-life care.

Role and Responsibilities:

  • Deliver health care services within patients' homes, assisted living facilities, and skilled nursing environments.
  • Perform light housekeeping and domestic tasks to maintain a comfortable living space.
  • Assist clients with personal care activities, ensuring their dignity and comfort.
  • Monitor patients' vital signs, including temperature and respiration, and report any changes in their condition.
  • Maintain accurate patient care records and document all services provided.
  • Support patients with mobility and prescribed exercises.
  • Educate and counsel patients and their families on dietary and exercise needs.
  • Provide companionship and emotional support to enhance the quality of life.

Qualifications:

  • Proven experience as a home health aide with valid HHA and CNA certifications.
  • Ability to monitor vital signs effectively.
  • Understanding of basic nutrition and personal hygiene standards.
  • Familiarity with end-of-life care practices, including post-mortem care.
  • Proficient in English, with the ability to read, follow written instructions, and document care accurately.
  • A caring and compassionate demeanor.
  • Current CPR certification and First Aid training.
  • Possession of a valid driver's license and a reliable, insured vehicle.
  • High school diploma or equivalent; bilingual Spanish-speaking candidates are strongly preferred.
