**Job Description**
We are seeking a highly skilled RN Care Manager to join our Partnership Program team. This rewarding opportunity is perfect for someone who thrives in a collaborative environment, values community case management, and shares our mission to support elderly individuals and adults with physical and intellectual disabilities. As a RN Care Manager, you will be responsible for:
  • Completing discipline-specific initial and ongoing assessments, conducting home visits, and consulting with families and other supports as needed.
  • Developing and maintaining care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports, and ensuring quality/coordination and effectiveness of services.
  • Completing all required documentation and charting, and assisting in the development and maintenance of member records.


**Required Skills and Qualifications**
To succeed in this role, you must possess:
  • An Associate degree in Nursing (required) or a Bachelor's Degree (preferred).
  • A current Wisconsin Licensed Registered Nurse license.
  • Eligibility for long-term care functional screener certification.
  • A valid WI Driver's License.
  • At least 2 years of recent experience working with older adults or individuals with physical or intellectual disabilities in a nursing capacity.
