Job Summary

We are seeking a skilled Registered Nurse to join our team as a Care Manager at the University of New Mexico Hospital. As a Care Manager, you will play a critical role in coordinating patient care and ensuring quality outcomes for our patients.

Key Responsibilities:

  • Coordinate patient care and develop comprehensive care plans
  • Collaborate with healthcare teams to ensure seamless transitions of care
  • Conduct timely discharge planning and coordinate post-acute care
  • Monitor and evaluate patient outcomes and make adjustments to care plans as needed
  • Communicate effectively with patients, families, and healthcare teams to ensure patient-centered care

Requirements:

  • 1 year of experience as a Registered Nurse
  • Current RN licensure in New Mexico or reciprocal agreement
  • CPR for Healthcare/BLS certification

Working Conditions:

This is a full-time position working days in a fast-paced hospital environment. The ideal candidate will be able to work independently and as part of a team to ensure high-quality patient care.
