Nurse Care Manager

We are seeking a skilled Registered Nurse to join our team as a Care Manager at the University of New Mexico Hospital. As a Care Manager, you will play a critical role in coordinating patient care and ensuring quality outcomes for our patients.
Key Responsibilities:
- Coordinate patient care and develop comprehensive care plans
- Collaborate with healthcare teams to ensure seamless transitions of care
- Conduct timely discharge planning and coordinate post-acute care
- Monitor and evaluate patient outcomes and make adjustments to care plans as needed
- Communicate effectively with patients, families, and healthcare teams to ensure patient-centered care
Requirements:
- 1 year of experience as a Registered Nurse
- Current RN licensure in New Mexico or reciprocal agreement
- CPR for Healthcare/BLS certification
Working Conditions:
This is a full-time position working days in a fast-paced hospital environment. The ideal candidate will be able to work independently and as part of a team to ensure high-quality patient care.
