We are seeking a skilled and compassionate Home Health Registered Nurse to join our team at Interim HealthCare. As a Home Health Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with a team of healthcare professionals to ensure the best possible outcomes.
Key Responsibilities- Provide comprehensive home-based care to patients with various medical conditions, including illnesses, injuries, and chronic diseases.
- Work collaboratively with physicians and other healthcare professionals to develop and implement individualized care plans.
- Conduct thorough assessments, document progress, and report changes to patients' physicians.
- Educate patients and their families on medication management, therapy, diet, and exercise.
- Coach family members on patient care, medication management, and home safety.
- Ensure patient goals are met and coordinate discharge from services.
- Active Registered Nurse (RN) License in Colorado.
- Home health care experience is preferred but not required.
- No Case Manager responsibilities.
- Graduate of an accredited school of nursing.
- At least 1200 hours of experience as an RN.
- Healthcare CPR/BLS Certification (demonstration course required).
- Pass a background check and drug test.
- Must be willing to travel to various locations in the area.
