At Trustbridge, we are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team as a Hospice LPN. As a Hospice LPN, you will play a vital role in providing end-of-life care to patients and their families.
Responsibilities:- Coordinate patient care under the direction of the primary physician and registered nurse.
- Identify patient and family needs and coordinate care accordingly.
- Contribute to the interdisciplinary group (IDG) and follow an individualized plan of care.
- Utilize monitoring, intervention, and teaching skills to maximize patient comfort and enhance quality of life.
- Current license to practice as a Licensed Practical Nurse in the State of Florida.
- Current Basic Life Support Certification.
- 30 hour IV Certification preferred.
- Hospice Certification preferred.
- Understanding of hospice nursing, case management, crisis intervention, communication skills, thanatology skills, and needs assessment skills.
- Bilingual (English/Spanish) preferred.
