Position Summary:
The Family Support Specialist plays a vital role in connecting families and youth with essential resources and referrals within a school environment. This position focuses on empowering participants to achieve personal objectives, particularly those aimed at securing stable housing. The program adopts a two-generation approach, emphasizing Parent Engagement, Economic Development, and Educational Workshops.
Key Responsibilities:
1. Assess and identify the strengths, needs, and risks of participants and their families to formulate both short- and long-term objectives related to housing stability and financial well-being.
2. Guide participants in developing individualized service plans that encompass necessary resources and services to support their goals.
3. Regularly update service plans in collaboration with participants and their families to ensure their needs are met and facilitate referrals to appropriate service providers.
4. Provide coaching to participants and their families to help them achieve personal goals, enhance life skills, and maintain success and wellness.
5. Conduct activities in a culturally sensitive manner, ensuring a supportive environment that maximizes participant success.
6. Maintain accurate records and manage data to track activities and outcomes effectively.
Additional Duties:
1. Support approximately 60 unique families and students annually.
2. Advocate for clients and families regarding their needs and access to services.
3. Prepare comprehensive documentation, including case notes and reports, utilizing computer applications.
4. Conduct client-centered social work activities in alignment with best practices.
5. Perform community and home visits as necessary to address client needs and monitor service delivery.
6. Stay informed about community resources and procedures for client referrals.
7. Facilitate case consultations for complex client situations.
8. Establish and review case plan goals with clients regularly.
9. Monitor client progress to ensure they are addressing identified challenges.
10. Provide crisis intervention and support services as needed.
11. Undertake additional responsibilities as assigned by the supervisor.
Qualifications:
1. Bachelor's degree in social work, psychology, or a related field.
2. Minimum of two years of experience in motivational interviewing, social service work, or community resource referral, preferably with individuals facing mental health challenges.
3. Knowledge of evidence-based practices, assessment processes, and outcome measurement.
4. Experience working with low-income families and youth is highly desirable.
5. Strong communication skills to engage effectively with diverse populations.
6. Ability to work both independently and collaboratively, maintaining confidentiality and discretion.
7. Proficiency in Microsoft Office Suite and related software.
