We are seeking a dedicated and compassionate Hospice Registered Nurse to join our team as a full-time RN Case Manager. This role involves managing and coordinating end-of-life care for patients in facilities and homes, working closely with doctors, caregivers, and family members.

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical, emotional, and psychological needs to develop personalized care plans.
  • Provide education and guidance to patients and their families about the dying process, care techniques, and available resources.
  • Maintain detailed records of patient care, treatment plans, and interactions, ensuring all documentation meets regulatory requirements.
