Job Summary

The Registered Nurse - Ambulatory Care Specialist provides and coordinates the care of patients/families and significant others in the ambulatory care setting by assessing, planning and intervening in actual or potential health problems, including evaluation of the patient's response to the care.

Key Responsibilities
  • Assess patient physical, psychosocial and spiritual health status and knowledge base to determine patient's health care and educational needs.
  • Provide and/or delegate patient care according to the care goals. Set priorities based on patient need.
  • Triage all sick and walk-in patients
  • Establish therapeutic and supportive relationships with patients and family.
  • Act as a resource person.
  • Coordinate client care including referral back to primary provider.
  • Assess needs and initiate referrals (VNS, Social Work).
  • Telephone triage.
  • Prescription requests, according to procedure.
  • Communicates normal and abnormal test results in collaboration with provider including appropriate followup.
  • Perform therapeutic treatments and procedures.
  • Assist with procedures.
  • Administer oral/IM/IV/SQ/Inhaled medications.
  • Coordinates delegates and/or provides all other patient care activities.
  • Coordinates in-patient admissions and ER referrals.
  • Acts as a patient advocate.
  • Documents according to standards of care.
  • Promotes and provides for patient safety in accordance with state and federal guidelines.
  • Participates in the department's performance improvement activities.
  • Provides presentations to care team members.
  • Ensures all staff participates as a member of the patient care team, participates in daily huddles and attend regular scheduled practice team meetings.
  • Attends in-service and continuing education programs.
  • Demonstrates a professional, courteous, and respectful attitude in dealing with patients, families and significant others.
  • Informs and educates patient/family to enable participation in their care and decision-making.
  • Evaluates the effectiveness of interventions and patient responses and outcomes to care.
  • Collaborates with team members to make the necessary changes to patient plan of care.
  • Communicates pertinent information and changes in patient status to appropriate members of the care team and other health care providers.
Requirements
  • BSN required.
  • Graduate from an accredited school of nursing with current New York RN registration.
  • BLS certification required (must be through the American Heart Association).
  • ACLS required (as of 6/1/21) for all RNs working in the Cancer Center or Center for Advanced Medicine & Surgery (CAMS)
  • Knowledge of current nursing, medical and health trends and practices in rendering of patient care
