About the Role

VNS Health is seeking a skilled Nurse Practitioner to join our team as a Float RN in the Home Health department. As a key member of our interdisciplinary care team, you will provide high-quality, patient-centered care to individuals in their homes and community settings.

Responsibilities
  • Practice independently in the community, delivering personalized nursing and care management to patients in their homes or care facilities.
  • Constantly evaluate evolving patient needs and respond with plan of care adjustments.
  • Collaborate with other healthcare professionals to ensure comprehensive, coordinated care.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Provide education and support to patients and their families to promote health and wellness.
Requirements
  • Current license to practice as a Registered Nurse in New York State.
  • Minimum of one year nursing experience in a medical/surgical environment.
  • Valid driver's license or NYS Non-Driver photo ID card may be required.
