Job Description:
The Licensed Practical Nurse (LPN) is responsible for providing direct patient care to residents in accordance with their individualized care plans. This includes administering medications, performing various clinical procedures, and maintaining accurate records of resident care.
Responsibilities:
- Administer medications and treatments as prescribed by physicians
- Monitor residents' health status and report any changes or concerns to the Resident Services Director
- Develop and implement individualized care plans in collaboration with interdisciplinary teams
- Provide education and support to residents and families on disease management and healthy living practices
- Maintain accurate and up-to-date medical records
Requirements:
- Licensed Practical Nurse (LPN) certification in the state of Connecticut
- 2+ years of experience in long-term care or gerontological nursing
- Ability to work effectively in a fast-paced environment and prioritize tasks
- Excellent communication and interpersonal skills
