Key Responsibilities

  • Service Delivery: Provide occupational therapy services to patients in their homes, ensuring compliance with prescribed physician orders and maintaining accurate records of patient progress.
  • Collaboration and Communication: Work closely with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure seamless care coordination and effective communication.
  • Documentation and Reporting: Maintain accurate and timely documentation of patient progress, including OASIS assessments, to ensure compliance with regulatory requirements.
Requirements
  • Education: Master's degree in Occupational Therapy from an accredited university/program.
  • Licensure: Current, unrestricted state license as an Occupational Therapist in the state of Arizona.
  • Experience: Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.
