As a Clinical Social Worker at Sutter Health, you will be working closely with interdisciplinary teams to provide comprehensive biopsychosocial assessments, develop personalized plans of care, and facilitate connections to community resources.

Job Responsibilities:

  • Perform thorough biopsychosocial assessments, identifying patients' medical, social, and emotional needs.">
  • Develop and implement individualized plans of care that address patients' concerns, strengths, and needs.">
  • Provide direct clinical services, including discharge planning, individual and family counseling, crisis intervention, and education and support groups.">
  • Facilitate communication among healthcare team members, patients, and families to ensure seamless transitions of care.">
  • Advocate for patients and families, promoting their rights and ensuring culturally competent care.">

Requirements:

  • ">
  • Masters in Social Work (MSW) degree from an accredited institution.">
  • Minimum 2 years of recent experience in clinical social work, preferably in a hospital or healthcare setting.">
  • Strong knowledge of community resources, transitions of care, and palliative care principles.">
  • Excellent communication, interpersonal, and problem-solving skills.">
  • Able to work effectively in a fast-paced environment with rapidly changing priorities.">
