JOB SUMMARY:

This position is responsible for providing case management services to adults with behavioral health problems and/or alcohol or drug issues in Washington County. Wraparound services include finding resources to assist in stabilizing housing, securing employment, furthering education, connecting with follow-up treatment and other areas of need in the person's life.

ESSENTIAL FUNCTIONS OF THE SUD CASE MANAGER:

  • Conduct an assessment to evaluate both the strengths and needs of the individual.
  • Analyze situations, capabilities, and problems to determine what services are required to meet the needs of the client.
  • Address the basic needs of the client, in addition to any therapeutic intervention, to increase effectiveness on issues such as housing, energy assistance, and job security.
  • Formulate, implement, evaluate, and revise goal-oriented and strength-based treatment plans.
  • Develop a crisis/safety plan for the clients.
  • Develop a support team of formal and informal supports, which will meet monthly to monitor progress of goals and services.
  • Research new and unknown resources and utilize that knowledge of services/resources to assist the client and family in meeting their goals.
  • Safely transport clients as needed.

LOCATION: Office is located in the Washington County Human Services Department with the opportunity to work hybrid/remote. This position requires local travel within the community and required to go into client homes, jails, and any other setting necessary to provide services.

SCHEDULE: Typical hours are Monday-Friday 8:00am-5:00pm.

SALARY: $25.00 per hour

JOB REQUIREMENTS:

  • Must have a Bachelor's Degree in Social Work, Psychology, Criminal Justice, or other related field.
  • At minimum, two years of previous experience working with individuals with mental health and addiction issues.
