Job Summary

As a Patient Engagement Coordinator, you will play a critical role in supporting patients throughout their treatment journey, providing a high level of care and attention to ensure their success.

Key Responsibilities
  • Engage Patients: Develop and implement individualized engagement plans to promote patient participation and motivation in treatment.
  • Provide Support: Offer emotional support and guidance to patients, addressing their physical, emotional, and psychological needs.
  • Coordinate Activities: Organize and lead group and individual activities, such as educational sessions, recreational activities, and community outings, to promote patient engagement and socialization.
  • Document Progress: Maintain accurate and detailed records of patient interactions, progress, and outcomes.
  • Collaborate with Staff: Work closely with treatment teams, including medical and clinical staff, to ensure comprehensive care and support for patients.
  • Conduct Crisis Intervention: Provide crisis intervention and support to patients experiencing emotional or psychological distress.
  • Develop Coping Skills: Teach patients coping skills and strategies to manage stress, anxiety, and other challenges.
  • Assist with Discharge Planning: Support patients in developing a plan for continued care and support after discharge.
Requirements
  • Education: High school diploma or equivalent required.
  • Experience: Previous experience working in a healthcare or social services setting, preferably in a substance abuse treatment facility.
  • Skills: Excellent communication and interpersonal skills, ability to work effectively with diverse populations, and strong organizational and time management skills.
  • Certifications: Certification in a relevant field, such as substance abuse counseling or crisis intervention, preferred.
