Job Summary:

Responsibilities:

  • Conduct monthly wellness visits for all residents and monitor their health status.
  • Document all pertinent information in the resident wellness file and maintain accurate records.
  • Communicate effectively with residents, families, and healthcare providers to ensure seamless care.
  • Participate in quality improvement initiatives and contribute to a culture of excellence in resident care.
Requirements:
  • Graduate of an approved college or school of nursing.
  • Current state license as a professional Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
  • Minimum of one (1) year experience in a long-term care or assisted living environment.
  • Strong knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.
  • Excellent communication and interpersonal skills.
