Nurse Specialist

Montefiore Medical Center is seeking a skilled Registered Nurse to provide high-quality care to patients in their homes.
Responsibilities:- Act as a liaison between patients, families, and physicians to ensure seamless care coordination.
- Provide updates on patient condition to family and physician, ensuring timely communication.
- Ensure all in-home medical equipment is in working order, promoting patient safety.
- Document patient details, progression, and regression activity, maintaining accurate records.
- Oversee physical therapy or rehabilitation exercises, promoting patient recovery.
- Contact emergency services when patients require hospitalization, prioritizing patient well-being.
- Possess knowledge of Home Care reimbursement processes and OASIS requirements.
- Have reliable transportation for home visits, ensuring timely care delivery.
- Ability to walk up/down six flights of stairs as needed, accommodating patient needs.
- Minimum one year hospital Med Surg experience, demonstrating clinical expertise.
- One year of Home Health experience or Certified Home Care (CHHA) experience, preferred.
- NYS RN License and Registration in good standing, ensuring professional credentials.
- Bachelor's degree, reflecting educational achievement.
- Infection Control certificate, demonstrating commitment to patient safety.
- Valid Driver License, Registration, and Automobile Insurance, ensuring safe transportation.
- Knowledge of a second language a plus, enhancing patient communication.
