Job Summary:We are looking for a dedicated and compassionate Registered Nurse to provide high-quality patient care in the comfort of our patients' homes. As a Registered Nurse - Home Health, you will work closely with our interdisciplinary team to deliver evidence-based care, promote patient safety, and improve health outcomes.
Responsibilities:- Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
- Collaborate with the clinical care team to develop and implement individualized care plans.
- Conduct virtual patient assessments, remote patient monitoring, and provide timely documentation of all nursing care.
- Participate in quality and safety education, rounds, and evaluations to ensure compliance with reporting requirements.
- Work closely with the pharmacy and care team to ensure medication reconciliation and education.
- Graduation from an accredited school of nursing.
- Current license to practice as a Registered Nurse in Massachusetts.
- Excellent clinical and interpersonal skills.
- Recent 3 years bedside nursing experience, preferably in Emergency and Medical/Surgical settings.
- Experience working in a start-up environment and/or willingness to adapt to continuous change.
- Telehealth and remote-patient monitoring experience preferred.
