Program Overview:

The Pennsylvania Institute for Community Living (PICL) operates a specialized 15-bed therapeutic community residential program designed to provide integrated self-help and recovery-oriented treatment. This program focuses on psychiatric rehabilitation, offering around-the-clock staffing and support for residents transitioning to permanent housing. The treatment approach encompasses all facets of Psychiatric Rehabilitation, with tailored lesson plans and group activities aimed at enhancing functional skills in areas such as social interaction, vocational training, financial management, emotional regulation, and coping strategies. Typically, residents engage in this program for a duration of 12 to 18 months to achieve their recovery goals.

Role Summary:

The Clinical Coordinator is tasked with organizing patient care initiatives and facilitating communication among all stakeholders involved in a patient's treatment. This role is pivotal in ensuring safe and effective treatment outcomes while embodying the core values of Resources for Human Development (RHD). The compensation for this position is competitive, with an hourly rate of $ per hour, translating to an annual salary of approximately $46,054.00.

Key Responsibilities:

  • Develop and monitor program initiatives.
  • Maintain familiarity with funding requirements, community support services, and relevant regulatory agencies.
  • Foster relationships with program participants, RHD staff, and external providers to ensure coordinated care.
  • Stay informed about services offered across the RHD service line.
  • Conduct regular reviews to analyze each client's care and connect them with available resources.
  • Create comprehensive treatment plans for clients requiring multiple services across different settings.
  • Ensure compliance with individual treatment plans.
  • Maintain a tracking system to identify the need for comprehensive treatment plans.
  • Engage with in-house and external partners through scheduled visits.
  • Facilitate regular collaborative planning meetings with the treatment team.
  • Provide data and outcomes related to the quality of care.
  • Respond to reporting requirements on a weekly and monthly basis.

Qualifications:

  • Master's Degree in a Human Services field.
  • 1-2 years of experience working with individuals with Mental Health Diagnosis, Intellectual and Developmental Disabilities, or Substance Use Disorders.
  • Possession of a valid Driver's License.

Physical Requirements:

  • Lifting Requirements: Ability to exert up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to move objects.
  • Physical Activities: Includes standing, walking, using hands for tasks, climbing, communicating, and visual tasks.

Reporting Structure:

  • Direct Supervisor: Unit Director
