We're seeking a dedicated healthcare professional to provide home-based medical care to complex, chronic patients. As a member of our interdisciplinary team, you'll work closely with primary care physicians, consultants, and other healthcare professionals to develop and implement patient care plans.

Your Key Responsibilities:

  • Conduct comprehensive assessments of newly enrolled patients and provide ongoing care thereafter
  • Develop and update patient management care plans as needed
  • Pursue preventative measures to avoid unnecessary acute admissions
  • Collaborate with hospital, emergency, or post-acute clinical teams following patient transfers
  • Educate patients and their families on disease processes and effective management strategies

Requirements:

  • Nurse Practitioner degree from an accredited program
  • Current state RN license and NP licensure
  • Access to reliable transportation
  • 2+ years of clinical experience preferred
