Mental Health Therapist for FAM Program

We are seeking a qualified In-Home Therapist to join our team at Stokes Counseling Services. As a Therapist for our FAM program, you will provide non-crisis outpatient level of counseling to clients in their homes.
Key Responsibilities:
- Conduct standardized counseling/talk therapy sessions for Stokes Counseling clients
- Perform intake/biopsychosocial assessments to evaluate clients' mental and behavioral health status
- Formulate differential diagnoses along with short-term and long-term goals
- Provide and maintain written documentation of intake, assessments, progress/contact notes, interventions, treatment plans, and termination notes within the designated time frame
- Maintain appropriate documentation of clinical and counseling services, including all necessary billing information
Requirements:
- Provisionally licensed mental health therapist in the State of Connecticut (LPCA, LMFTA, or LMSW)
- 1-3 years experience conducting counseling/talk therapy sessions with diverse client populations
- Passion for in-home work
- Ability to work collaboratively in a team-oriented environment
- Advanced proficiency in conducting intake assessments and formulating treatment plans
- Strong written communication and interpersonal skills with the ability to provide and maintain documentation within the appropriate time frames
- Have a valid driver's license and access to a properly insured vehicle for work or reliable transportation
