As a Speech Pathologist Home Health, you will have the opportunity to work with patients of all ages, from neonates to geriatrics, and provide expert care in a variety of settings.

Our team is dedicated to delivering exceptional patient care, and we are seeking a skilled and compassionate Speech Pathologist to join our Home Health team.

Responsibilities:

* Provide patient care for neonate, pediatric, adult, and geriatric patients
* Meet discipline-specific productivity standards
* Demonstrate competency in care of patients during field observation by the clinical supervisor
* Complete and submit documentation following established guidelines
* Ensure physician orders are present and written correctly
* Document follow-up of identified problems
* Review and document clinical progress toward goals
* Demonstrate coordination of care by appropriate communication with other disciplines and team members
* Meet agency/regulatory requirements
* Document appropriate management of visit utilization and authorization status

Qualifications:

* Current, valid Speech-Language Pathologist (SLP) license from the California Board of Medical Quality Assurance
* Two (2) years of experience within a healthcare setting
* Current Basic Life Support (BLS) certification through the American Heart Association (AHA)
* Current California driver's license, possession of automobile insurance and access to an operating motor vehicle
