About the Role

We are seeking a skilled Speech Pathologist to join our Home Health team at Dignity Health Woodland Memorial Hospital. As a Speech Pathologist, you will play a vital role in providing high-quality patient care and improving the lives of our patients and their families.

Responsibilities
  • Provide patient care for neonate, pediatric, adult, and geriatric patients, demonstrating competency in care and meeting productivity standards.
  • Complete and submit documentation following established guidelines, ensuring accurate and timely reporting.
  • Ensure physician orders are present and written correctly, and document follow-up of identified problems.
  • Review and document clinical progress toward goals, demonstrating coordination of care through effective communication with other disciplines and team members.
  • Meet agency and regulatory requirements, documenting management of visit utilization and authorization status.
  • Meet regulatory requirements for supervisory visits, ensuring compliance with established guidelines.
Qualifications
  • Current, valid Speech-Language Pathologist (SLP) license from the California Board of Medical Quality Assurance.
  • Two (2) years of experience within a healthcare setting.
  • Current Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • Current California driver's license, possession of automobile insurance, and access to an operating motor vehicle.
