Role Overview:
The LPN Team Leader for Memory Care and Assisted Living serves as the cornerstone of our community, responsible for optimizing staffing levels within established guidelines. This role includes supervising and guiding the development and execution of service plans through effective leadership of care aides and medication assistants. The LPN Team Leader will also ensure compliance with health regulations and internal policies, maintaining a focus on resident safety and care quality.
Key Responsibilities:
- Conduct interviews, hire, and onboard new direct care staff, ensuring a supportive training process.
- Perform 90-day and annual performance evaluations, implementing improvement plans as necessary.
- Assist in the planning, organization, and evaluation of resident care services, ensuring adherence to regulations.
- Collaborate with administration and medical staff to coordinate resident care effectively.
- Maintain accurate service records and resident documentation with the support of the Clinical Leader.
- Foster positive relationships with residents and their families, supporting the care team in this endeavor.
- Report any medication errors and address resident grievances promptly.
- Promote a warm and inviting atmosphere, emphasizing personal interest and positivity.
- Provide mentorship and training for new or less experienced staff members.
- Identify areas for service improvement and assist in implementing solutions.
- Ensure compliance with safety and infection control policies among staff and residents.
Qualifications:
- LPN certification preferred or a commitment to obtaining licensure.
- Minimum of 1-2 years of experience in a caregiving role within a healthcare or assisted living setting.
- Experience with medication administration is required; supervisory experience is a plus.
- High school diploma or GED is mandatory.
