PACE Southeast Michigan Nurse Care Manager

The Registered Nurse Case Manager plays a critical role in utilizing a systematic approach to nursing practice, incorporating assessment, planning, implementation, and evaluation of care for frail elders with complex needs.

This position involves leading or directing licensed and non-professional nursing staff in the coordinated delivery of care to participants of the PACE Southeast Michigan program.

Key Responsibilities:
  • Assess participants' needs and plan for appropriate nursing care upon Initial Intake Assessment and routine Re-Evaluation Assessments.
  • Collaborate with participants, families, and the multidisciplinary Team to develop participant plans of care.
  • Maximize participants' functional capacity by encouraging autonomy in all aspects of care.
  • Teach, supervise, and counsel participants, caregivers, or family members regarding nursing care needs and other related problems.
  • Initiate preventative and rehabilitative procedures or programs as appropriate for participants' care and safety.
  • Administer medications and treatments as ordered by the physician/NP, and monitor participants' responses.
  • Demonstrate knowledge of administered medications and instruct participants/families in safe administration in the home.
  • Recognize and understand the significance of abnormal test results and utilize critical thinking skills when gathering participant data, planning for, and implementing care.

We offer a competitive salary of $ $63,000 - $85,000 per year, depending on experience.
