About the Role:

\

We are seeking an experienced Registered Nurse to join our after-hours hospice care team. In this vital role, you will advocate for your patients and coordinate closely with the patient, their families, and our hospice care team.

\

The ideal candidate will have a minimum of 2 years of nursing experience in a clinical care setting, with hospice experience preferred. Key Responsibilities:

  • Advocate for patients and coordinate care with the hospice care team
  • Provide exceptional care and support to patients and their families
  • Collaborate with the patient, their families, and our hospice care team to develop and implement personalized care plans

Requirements:

  • Graduate of an accredited nursing program and hold a current state RN license
  • Minimum of 2 years of nursing experience in a clinical care setting; hospice experience preferred
  • Reliable transportation and the physical ability to perform the tasks of the job
\
