Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at PACE Southeast Michigan. As an LPN, you will play a vital role in providing high-quality care to our participants, working collaboratively with our interdisciplinary team to develop and implement individualized plans of care.

Key Responsibilities
  • Collect and document assessment data to inform care planning and evaluation
  • Assist primary care providers in collecting specific assessment data and implementing care plans
  • Administer medications as ordered by physicians, ensuring safe and accurate administration
  • Communicate effectively with participants, families, and healthcare team members to ensure seamless care coordination
  • Participate in ongoing education and training to enhance knowledge and skills
Requirements
  • Current Michigan licensure as a Licensed Practical Nurse
  • One year of experience working with frail or elderly populations
  • Ability to establish and maintain effective interpersonal and interdisciplinary relationships
  • Medical clearance for communicable diseases and up-to-date immunizations
Working Conditions

This role requires manual dexterity, walking, standing, reaching, bending, and lifting of up to 40 pounds. You will work in a variable environment with exposure to communicable diseases.
