Registered Nurse II, Ambulatory Care Specialist

We are seeking a highly skilled Registered Nurse II to join our Ambulatory Care team. As a key member of our team, you will be responsible for delivering high-quality patient care and contributing to the development of our innovative culture.
Key Responsibilities- Assess, plan, deliver, coordinate, integrate, and evaluate nursing care related to patient needs.
- Contribute to the support of patients through interdisciplinary teamwork and provide nursing care and services to patients throughout the life span and in various settings.
- Collect and record relevant patient health data accurately and work in collaboration with the care team to contribute to the plan of care.
- Analyze assessment data to identify patient problems and utilize data to continuously meet care coordination needs.
- Identify expected outcomes individualized to the patient and evaluate patient response to plan of care.
- Implement and record interventions appropriate to diagnosis or patient problem and evaluate the patient's progress toward attainment of identified goals.
- Adhere to approved, research-based practice standards and direct and supervise patient care provided by LPNs and support personnel.
- Perform and document Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures.
- Maintain and document testing reagents/supplies and equipment according to established laboratory procedures.
- Act effectively as a group leader or preceptor when assigned and advocate for patients to ensure that care preserves and protects the patient's autonomy, dignity, and rights.
- Deliver care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity.
- Education: 2-year/Associate Degree in Nursing, required or Vocational/Technical Diploma Professional Nursing Program, required or 4-year/Bachelor's Degree in Nursing, preferred.
- Experience: Minimum one year of experience as an Registered Nurse, required.
- License/Certification: Current NYS license as a Registered Nurse, required.
- BLS certification will be required within 6 months.
