Position Summary
The Wound Care Licensed Practical Nurse (LPN) plays a critical role in delivering and coordinating patient care. This includes gathering patient histories and wound assessments to assist in developing effective therapy plans under the supervision of a healthcare provider or registered nurse. The LPN also provides education to patients and their families regarding their care plans.
Key Responsibilities
- Exhibit clinical proficiency in a clinical setting, focusing on comprehensive wound assessments, patient care coordination, and the formulation and implementation of therapy plans, alongside patient and family education.
- Handle multiple priorities while managing a significant caseload of complex wound patients, regularly reviewing therapy plans to ensure optimal healing outcomes.
- Collaborate with physicians, patients, and healthcare team members to strategize patient care and treatment, demonstrating competence in guiding the implementation of therapy plans.
- Stay informed about all regulatory and hospital policies, including safety protocols, hazardous waste management, and emergency procedures.
- Integrate knowledge from research and educational opportunities into current care standards and policy updates.
- Pursue advanced knowledge and credentials to enhance clinical performance, serving as a mentor and role model for peers.
- Actively engage in initiatives aimed at improving the work environment and patient care, supporting Quality Improvement processes.
- Participate in daily operations through attendance at staff meetings and related activities.
- Travel to various system wound care locations as required, with travel expenses reimbursed.
- Perform additional duties as assigned.
Qualifications
Education: Completion of an accredited practical nursing program with coursework in medication administration.
Skills: Ability to respond to critical situations with professionalism, demonstrating sound clinical judgment and effective communication with all healthcare team members. Commitment to ongoing professional development and proficiency in computer systems.
Experience: A minimum of 2 years of clinical experience is required, with ambulatory experience preferred.
Licensure: Current State Licensed Practical Nurse license.
Certification: CPR certification is mandatory.
Preferred Qualifications
Education: Completion of an accredited LPN program.
Experience: Background in wound care management is highly desirable.
Licensure: Dual licensure in Ohio and Michigan is preferred.
Certification: Additional certifications in wound care, diabetes management, or vascular disease are advantageous.
Working Conditions
Must be capable of moving throughout the facility and between workstations, enduring prolonged periods of standing, and managing exposure to various environmental factors.
