About the Role

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team as a Visiting Nurse. As a key member of our interdisciplinary team, you will provide high-quality patient care in the comfort of patients' homes, working closely with physicians and other healthcare professionals to develop and implement individualized care plans.

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions to address these needs.
  • Develop and implement individualized care plans, working collaboratively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
  • Collaborate with interdisciplinary teams to ensure comprehensive care, including social work, therapy, and other services as needed.
  • Document patient progress, including assessments, interventions, and outcomes, to ensure accurate and timely reporting.
Requirements
  • Current RN licensure in North Carolina or compact RN licensure.
  • Two years of med-surgical, critical care, or home health/hospice/infusion experience.
  • Valid driver's license and current automobile insurance coverage.
  • Prior experience in Home Health, Hospice, or Infusion preferred.
  • Prior OASIS and PDGM experience preferred for home health.
