We are seeking a skilled and compassionate LVN to join our team at The Elizabeth Hospice. As a Continuous Care LVN, you will provide professional nursing care to patients in their homes or facilities, working under the supervision of an RN. This is a full-time position requiring 72 hours per pay period, with a competitive salary range of $30-$38 per hour.
Responsibilities- Assist RN in analyzing situations, anticipating problem areas, and taking appropriate action.
- Evaluate patient response to care, ensuring safe and timely outcomes.
- Perform licensed nurse skills, adhering to policies and procedures, standards of care, and critical competencies.
- Implement and document teaching plans, tailored to patient and family needs.
- Document all care provided, including telephone interactions with staff members, using electronic charting.
- Participate in team meetings, collaborating with RNs and other healthcare professionals.
- Vocational Nurse Training.
- Minimum 12 months of LVN experience, preferably in acute care, home health, or hospice.
- Current California LVN license.
- Familiarity with computerized bedside patient documentation.
- Ability to work as part of a team, taking direction from RNs and other team members.
- Phlebotomy certification and bi-lingual skills are a plus.
