Compassionate Hospice Care Specialist

We are seeking a compassionate and dedicated LPN to join our team as a Hospice Care Specialist. In this role, you will provide individualized care to patients in their homes and facilities, working under the direction of an RN.
Responsibilities- Provide quality nursing care to patients and their families
- Work collaboratively with the interdisciplinary team to develop and implement patient care plans
- Administer medications and treatments as ordered by the physician
- Monitor patient vital signs and report any changes to the RN or physician
- Registered LPN licensure in the State of Ohio
- At least one year of nursing experience preferred
- CPR certification
- Computer skills sufficient to document services and care
- Able to drive during daytime, nighttime, or inclement weather
- $55,000 - $65,000 per year, depending on experience
