Job Summary

We are seeking a skilled Hospice Registered Nurse to join our Home Health team. As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient care and collaborating with healthcare providers to ensure seamless transitions between care settings.

Key Responsibilities
  • Assess and develop patient-specific plans of care, ensuring timely and effective delivery of services.
  • Collaborate with healthcare providers, other disciplines, and communicate changes promptly to ensure continuity of care.
  • Educate and instruct patients/caregivers in ongoing medical maintenance, beginning discharge planning on admission.
  • Complete all documentation within 24 hours, including changes in condition, follow-up with providers, and supporting documentation.
  • Evaluate patient progress and take prompt action to address any concerns.
  • Oversight of LPNs and Home Health Aides, ensuring adherence to policies and procedures.
  • Participate in case management conferences and document according to guidelines.
  • Schedule patients independently according to individual plans of care.
  • Attend all staff meetings, inservices, and other required meetings, ensuring compliance with mandatory requirements.
Requirements
  • Nursing Diploma or Associates Degree in Nursing; Bachelors in Nursing preferred.
  • Two years nursing experience preferred; previous home care experience desirable but not required.
  • Registered Nurse with Valid (or eligible) Connecticut license.
  • Excellent clinical knowledge, verbal/written skills, and organizational skills, with ability to work independently.
  • Strong ability to work with providers and members of the multidisciplinary team.
Additional Information

Home care experience required, hospice preferred. Current RN license and driver's license required. Coverage area: Branford or north/west territory. Shift: Evening/Nights/Weekends.
