We are seeking a skilled Registered Nurse to join our Home Health team. As a Home Health Behavioral Health Registered Nurse, you will be responsible for providing high-quality patient care and collaborating with healthcare providers to ensure seamless care transitions.

Key Responsibilities
  • Assess and reassess patients to develop individualized plans of care.
  • Collaborate with healthcare providers and other disciplines to ensure comprehensive care.
  • Ensure ongoing authorizations and payment sources are in place.
  • Provide education and instruction to patients and caregivers on ongoing medical maintenance.
  • Document all changes in patient condition and follow-up with healthcare providers as needed.
  • Evaluate patient progress and take prompt action as necessary.
  • Oversight of LPNs and Home Health Aides.
  • Participate in case management conferences and document according to guidelines.
  • Schedule patients independently according to individual plans of care.
  • Attend all staff meetings, inservices, and other required meetings.
  • Comply with all mandatory requirements.
Requirements
  • Nursing Diploma or Associates Degree in Nursing.
  • Two years of nursing experience preferred.
  • Previous home care experience desirable but not required.
  • Registered Nurse with a valid Connecticut license.
