Job Title: Clinical Coordinator

Community Behavioral Health (CBH) is seeking a highly skilled Clinical Coordinator to join our team. As a Clinical Coordinator, you will play a critical role in directing the daily operations of our clinical teams, ensuring that our standards of care and practices are delivered in accordance with state regulation guidelines.

Key Responsibilities:
  • Team Leadership: Build and maintain a strong functional team through effective recruiting, training, performance management, coaching, team building, and succession planning.
  • Policy Development: Create policy and protocols to help the Clinical Department function effectively.
  • Clinical Quality Auditing: Perform and oversee clinical quality auditing and feedback.
  • Productivity Monitoring: Monitor productivity and ensure that standards are being met.
  • Supervision: Provide regular supervision and documentation of supervision consistent with standards in the field.
  • Collaboration: Participate in internal and external meetings to develop, monitor, and analyze services and resources to ensure they effectively and efficiently meet the needs of members.
  • Community Engagement: Ensure that Clinical Care Managers have ease of access to provider sites to conduct work with members, families, and providers.
  • Clinical Supervision: Accompany Clinical Care Managers and Clinical Supervisors to community between 10 and 15% of work time to provide ongoing clinical supervision and feedback.
  • Clinical Consultation: Meet with staff to provide clinical consultation on cases.
  • High-Risk Member Support: Work with Physician Advisors to ensure appropriate consultation for high-risk members.
  • Crisis Management: Anticipate and manage response of crisis situations.
  • Interdepartmental Collaboration: Work closely with Clinical Care Management Coordinators of other teams to facilitate consistent longitudinal care management.
  • Quality Improvement: Work collaboratively with the Quality Management Department to identify key indicators of provider stability and ensure monitoring and action to rectify any concerns.
  • Interdepartmental Efficiency: Maintain collaboration with other CBH departments to ensure intradepartmental efficiencies.
  • Communication: Ensure interdisciplinary communication of critical member and provider-related information.
  • Data Analysis: Obtain and review data relevant to members served by the team, clinical operations, and staff monitoring to guide interventions.
  • Evidence-Based Practices: Ensure that direct reports have adequate knowledge of evidence-based practices and which evidence-based practices are offered by providers in network; make certain that evidence-based practices are routinely discussed in supervision between Clinical Supervisors and Clinical Care Managers.
  • Data-Driven Decision Making: Utilize data-driven decision making.
  • Customer Service: Model and require exceptional customer service in work with members and providers.
  • On-Call Duties: Perform on-call duties as instructed.
  • Additional Responsibilities: Perform other duties as assigned.
Team-Specific Responsibilities:
  • Program Development: Develop and oversee operations of the clinical specialty programs.
  • Care Management Approach: Continues to develop and design care management and longitudinal care coordination approach for the team and considers opportunities for expansion and growth.
  • Provider Relationships: Builds relationships with behavioral and physical health providers to develop protocols for communication and collaboration.
  • System Partnerships: Collaborates with other system partners (i.e., Community-Based Organizations, PH-MCOs, DBHIDS) to address member's Whole Person-Centered Care based on evidence-based practices.
  • Strategic Interventions: Focus on strategic targeted interventions designed to decrease readmissions and service utilization.
  • Data-Driven Interventions: Uses data to identify members and cohorts with specific or increased community-based behavioral health needs and develops protocols to ensure members are connected with appropriate supports, including Evidence-Based Practices (EBPs) when possible.
  • Integrated Care Coordination: Collaborate closely with the Integrated Care Plan (ICP) team, as well as other teams throughout the department to promote whole-person integrated care coordination.
Position Requirements:
  • Education: Master's degree in Social Work or related field.
  • License/Certification: License preferred; progress towards licensure within an agreed-upon time frame as a condition of continued employment. PA Child Abuse Clearance required.
  • Relevant Work Experience: Minimum of 3 years of post-graduate experience with a minimum of 1 year of supervisory experience.
Skills:
  • Verbal and Written Communication: Strong verbal and written communication and presentation skills.
  • Utilization Reports: Able to read, analyze, and interpret utilization reports.
  • Mental Health and Substance Abuse Services: Understanding of mental health and substance abuse services or interventions.
  • Problem-Solving Skills: Strong problem-solving skills.
  • MS Office Proficiency: Proficient in MS Office and able to type at least 20 words per minute.
  • Teamwork: Able to work collaboratively in team settings.
  • Organizational Skills: Strong organizational skills.
  • HIPAA Compliance: Compliant with HIPAA regulations.
  • Clinical Skills: Strong clinical skills.
  • Clinical Supervision: Ability to provide clinical supervision related to complex clinical presentation of member.
  • Critical Thinking and Problem-Solving: Critical thinking and clinical problem-solving abilities.

  • Strategic Thinking: Understand and integrate the long-term vision of CBH and focus current activities on what is critical to achieve future success, including formulating objectives/priorities and developing and implementing breakthrough ideas.

Work Conditions:
  • Travel Requirements: Requires ability to travel, including out-of-state and overnight, as needed.
  • Physical Demands: Regularly requires walking, standing, stooping, bending, sitting, reaching, pushing, pulling, and/or repetitive wrist/hand movements for various lengths of time throughout the day.
  • Computer and Office Equipment: Ability to utilize computers and office equipment to complete daily work responsibilities.
  • Work Environment: Regularly works inside in areas that are adequately lighted and ventilated. Some fluctuation in temperature. No protective equipment routinely needed.
  • Adaptability: Must be able to adapt to continuous changes/demands of the job.
Philadelphia Residency Requirement:
  • Residency Requirement: The successful candidate must be a current Philadelphia resident or become a resident within six months of hire.
