About the Role:

We are seeking a skilled and compassionate Certified Home Health LPN to provide high-quality patient care in the comfort of their own homes. As a member of our team, you will have the flexibility to create your own schedule and work at your own pace.

Responsibilities:
  • Provide one-on-one skilled nursing care and treatment to patients in their homes.
  • Assess patients' conditions and contribute information to aid in planning and evaluating care.
  • Collaborate with a multidisciplinary care team to develop an individualized plan of care that achieves desired outcomes and goals.
  • Document patient information accurately and timely, according to industry standards and guidelines.
Requirements:
  • Current and unencumbered license as a Licensed Practical Nurse (LPN).
  • Strong assessment, critical thinking, and communication skills.
  • 1+ year(s) of Certified Home Health experience as an LPN.
  • Ability to work independently and as part of a dynamic team.
