Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will work independently to assess, plan, and implement individualized care plans to help our clients achieve their rehabilitation goals.

Responsibilities:
  • Conduct thorough assessments of clients' physical abilities and develop personalized care plans to address their needs.
  • Provide one-on-one physical therapy sessions to clients in their homes, focusing on exercises, stretches, and other interventions to improve mobility, strength, and function.
  • Collaborate with other healthcare professionals, including nurses and occupational therapists, to ensure comprehensive care and coordination.
  • Document client progress, update care plans, and communicate with clients, families, and healthcare teams as needed.
  • Stay up-to-date with industry developments, best practices, and evidence-based guidelines to provide the highest level of care.
Requirements:
  • A current NJ Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently, manage time effectively, and prioritize tasks.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with experience using electronic medical records (EMR) a plus.
