Job Summary

Eden Health is seeking a skilled and compassionate Registered Nurse to provide high-quality in-home care to our patients in Snohomish county. As a key member of our team, you will be responsible for delivering admission, case management, and follow-up skilled nursing visits to our patients, ensuring they receive the best possible care and support.

Responsibilities
  • Provide hands-on care, management, and evaluation of patient care plans, teaching patients and their families as needed.
  • Coordinate care with various healthcare professionals, including physical therapists, occupational therapists, and social workers.
  • Document patient care, condition, and progress, reporting to physicians and clinical managers as required.
  • Implement patient care plans in collaboration with patients and their families, ensuring optimal resolution of needs and problems.
  • Supervise and coordinate the work of Home Health Aides, providing written instructions and guidance.
  • Prepare and complete necessary clinical documentation, including case conferences, patient contacts, and care plan changes.
Requirements
  • Graduate of an accredited nursing program.
  • Current licensed Registered Nurse (RN) in the state of practice.
  • Experience as an RN in an acute-care setting or home health environment, with preferred credentials in specialty areas such as wound care, OASIS certification, IV, and ET.
  • Valid driver's license, reliable transportation, and proof of automobile insurance.
  • Proof of current CPR certification and ability to pass a criminal background check and national sex offender clearance.
