Job Summary:The Licensed Clinical Social Worker will provide direct clinical services, including assessment, treatment planning, individual, family, and group psychotherapy, crisis counseling, and community consultation and education. You will work closely with our multidisciplinary team to ensure seamless care coordination and excellent patient outcomes.
Responsibilities:- Provide culturally sensitive direct clinical services, including assessment and treatment planning, individual, family, and group psychotherapy, crisis counseling, and community consultation and education.
- Document client progress to maintain a permanent record of client activity according to established methods and procedures.
- Meet monthly productivity targets, contingent on the number of hours worked.
- Submit progress notes within 72 hours of services, preferably on the same day of service.
- Identify case management needs of clients and coordinate with other agency staff and community agencies to meet these needs.
- Perform crisis intervention as necessary.
- Coordinate treatment with other community agencies and services.
- Participate in program development and training programs.
- Follow established clinic policies and procedures.
- Maintain appropriate professional standards.
To be successful in this role, you will need:
- A Master's degree in Social Work (MSW) or a related field.
- Licensure as a Licensed Clinical Social Worker (LCSW) in the state of New York.
- Experience working in an outpatient setting, preferably in a community health center.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Commitment to providing high-quality, patient-centered care.
Sun River Health offers a competitive salary range of $65,000 to $100,000 per year, depending on licensure and experience. We also offer a retention bonus, regular clinical supervision, and loan forgiveness opportunities for LCSWs through the HRSA program.
