About the Position:
At Accompany Health, Advanced Practice Clinicians play a pivotal role within our Integrated Care Teams, which encompass Physicians, Community Health Workers, Patient Experience Navigators, RNs, Social Workers, Behavioral Health Clinicians, Psychiatrists, and Pharmacists. This collaborative team is dedicated to delivering and coordinating comprehensive, patient-centered care for a select group of patients facing intricate medical, behavioral health, and social challenges. As an Advanced Practice Clinician in our innovative care model, you will provide ongoing care and foster trusting, therapeutic relationships with your patients. You will deliver care directly to patients—either independently or in partnership with physicians—across home, community, and virtual settings. For your designated patient panel, you will ensure thorough chronic disease management, exceptional preventative care, and timely urgent care as necessary. Our smaller patient panels, combined with extensive support from a multidisciplinary team, allow you to dedicate more time to each patient, significantly enhancing their health and well-being.

Key Responsibilities:

  • Deliver outstanding patient-centered and holistic primary care through home or community-based visits, as well as virtual consultations.
Develop comprehensive and suitable clinical care plans for your patient panel by actively participating in and leading multidisciplinary case conferences.
Collaborate with external healthcare providers and community organizations to advocate for patient care that aligns with their personal goals.
Conduct annual wellness visits and patient evaluations to gain a complete biopsychosocial understanding of patients' health, focusing on precise documentation and addressing quality care gaps.
Provide clinical coverage as necessary, including home-based follow-ups and urgent visits when appropriate.
Participate in a rotating call schedule to meet patients' urgent clinical needs.
Ensure timely and accurate documentation within our electronic medical record system and our custom-built platforms that facilitate patient care.
Engage in the recruitment and training of fellow advanced practice clinicians.

Required Skills and Experience:
Mandatory:
Active Nurse Practitioner license in the state of Michigan.
AANC or AANP board certification for Nurse Practitioners.
Active DEA Controlled Substance Registration or eligibility for application. X-waiver licensure (for buprenorphine prescribing) is a plus but not required.
Active controlled substance license in the state of employment or eligibility for application.
Certification in Basic Life Support for Healthcare Providers.
Minimum of 2 years of clinical experience in primary care within internal medicine, family medicine, or geriatric medicine settings.
Proven skills, knowledge, and judgment necessary for independent clinical assessment and decision-making regarding acute and non-acute clinical issues.
Proficiency and experience in working with electronic medical records.
Valid unrestricted driver's license and access to an insured vehicle for daily use.
Preferred:
Experience in adult internal medicine, family medicine, geriatrics, addiction medicine, and palliative care.
Experience in providing home-based care.
Passion for caring for individuals with complex chronic medical, behavioral, and social needs.
Experience engaging individuals with untreated and/or symptomatic chronic mental illness and addiction.
Prior experience as part of a multidisciplinary care team.
Familiarity with value-based care organizations.
