Job Summary

We are seeking a highly skilled Referral Specialist to join our team at Family Health Centers of San Diego. As a Referral Specialist, you will play a critical role in ensuring seamless patient care by accurately completing referrals, tracking follow-up, and communicating with clinic staff.

Key Responsibilities
  • Accurately complete referrals, including ICD-9 and CPT codes, following established department protocols.
  • Track follow-up and referral pool(s), scheduling patient services as required.
  • Communicate referral status changes to clinic staff using CMIS updates and chart notes.
  • Enter required data, including basic insurance and screening information.
  • Follow established protocols for patients in crisis.
  • Make assigned reminder calls.
  • Submit accurate and timely data about referrals and appointments by site.
Requirements
  • High school diploma or GED equivalent required.
  • Successful completion of a Medical terminology course preferred.
  • 1 year of experience in an ambulatory care setting required.
  • Or equivalent combination of education and experience that provides the skills, knowledge, and ability to perform the essential job duties.
  • Bilingual in Spanish/English preferred.
