Position Overview

In the role of a Senior Transitional Care Assistant, you will serve as a crucial guide, educator, and supporter, empowering clients with essential knowledge and resources for effective transitions in their care. You will act as a vital link between clients and the healthcare team, while also delivering administrative and customer service assistance.

Key Responsibilities

  • Engage with clients to understand their healthcare and functional objectives, ensuring a smooth transition to home or other care settings.
  • Collaborate closely with the Care team to monitor client progress, ensuring accurate and timely documentation.
  • Facilitate regular case record reviews and discussions with the Care team for assigned clients.
  • Establish and nurture collaborative relationships with caregivers, hospital staff, physicians, nurses, discharge planners, case managers, social workers, and other personnel.
  • Document all interactions with the interdisciplinary team in the electronic medical record (EMR).
  • Follow up with clients and adhere to expected timelines as per established workflows.
Qualifications

Licenses and Certifications:

  • Valid driver's license or NYS Non-Driver photo ID card may be required based on operational needs.

Education:

  • Associate's degree in human services or a related field, or equivalent work experience, required.
  • Bachelor's degree in a human services or related field, preferred.

Work Experience:

  • Minimum of two years of customer service experience in the medical field (e.g., hospital, long-term care, home care, medical office), required.
  • Ability to adapt to change and work effectively in a team-oriented environment, required.
  • Strong follow-up skills and the ability to manage multiple priorities, required.
  • Proven ability to educate clients, conduct phone interviews/coaching, and coordinate services for multiple patients with chronic conditions, required.
  • Organizational skills and attention to detail are essential.
  • Experience with data entry and navigating computer systems, required.
  • Proficiency in Microsoft Office is preferred.
  • Bilingual skills may be necessary based on operational requirements.
